Dependents Gaining Other Coverage
Dependents gaining other coverage will have 31 days from the date their new coverage takes effect to submit the enrollment/change forms to the Williamson County Benefits Department.   A Certificate of Coverage (COC) from the dependent’s new insurance provider(s) reflecting the date their coverage became effective needs to be submitted with the enrollment/change forms.

Dependents Losing Other Coverage
Dependents losing coverage will have 31 days from the date their coverage was terminated to submit the enrollment/change forms to the Williamson County Benefits Department.  A Certificate of Coverage (COC) from the dependent’s previous insurance provider(s) reflecting the date their coverage was terminated needs to be submitted with the enrollment/change forms.  A termination notice from the dependent’s previous employer is NOT acceptable.

If applicable, the employee, spouse, and children may have to complete the Evidence of Insurability Form for enrollment of Supplemental Life Insurance.

Note: All dependents are eligible for Life Insurance with evidence of insurability. The employee has to be enrolled in Supplemental Life in order for the dependents to be eligible for Supplement Life Insurance – Spouse/child(ren).
